Shift Pattern: 4:3 rota (Days & Nights: 8am–8pm / 8pm–8am)
Driving Licence: Essential

This is a dynamic role where no two days are the same. You’ll be at the heart of operations, ensuring care is delivered safely, efficiently, and to the highest standards.


What You’ll Be Doing

  • Acting as a key point of contact for carers, nurses, and service users, ensuring smooth day-to-day coordination
  • Creating and managing staff rotas, ensuring care is delivered effectively and consistently
  • Developing and updating person-centred care plans in response to changing needs
  • Supporting and guiding care staff to deliver high-quality care
  • Participating in the on-call rota, providing out-of-hours support when required
  • Proactively identifying and resolving issues to maintain continuity of care
  • Conducting service user visits to monitor care quality and compliance
  • Ensuring all care delivery meets health & safety and regulatory standards
  • Building strong relationships with families, professionals, and local stakeholders
  • Monitoring care delivery, producing reports, and maintaining accurate records

What We’re Looking For

  • NVQ Level 2 or 3 in Health & Social Care (or willingness to work towards)
  • Previous experience in care coordination or a similar role
  • Confident managing rotas and care schedules (experience with rota software preferred)
  • Strong communication and interpersonal skills
  • Ability to stay calm under pressure and solve problems effectively
  • Highly organised with excellent attention to detail
  • IT literate (Microsoft Office and care systems)
  • A full UK driving licence and access to a vehicle
